Final Clinical Impression

Lucille Palmeiro, a 38 year old Caucasian female, presented to the Emergency Room of Princeton-Plainsboro Hospital, after having a respiratory arrest, as a sequale of pulmonary embolism. The pulmonary embolism resulted from a deep vein thrombosis (DVT)/ blood clot originating from her calf.
Blood alcohol of 0.12 early in the morning (? alcoholic).
History of auditory hallucination and paranoid ideation (? schizophrenia)
Ultrasound scanning showed a 5.8 solid mass in her liver (cancerous), which was then surgically resected. Further assessment revealed Kaiser-Fleischer rings, copper-colored circles around the corneas, confirming the diagnosis of Wilson’s Disease.

History
Presenting Hx: Respiratory arrest due to pulmonary embolism, originating from DVT in the calf.
Tremor.
Hemoptysis/ coughing up blood (Blood clotting problem?)
Past Hx: Hx of repeated auditory hallucinations and paranoid ideations.
Self-harming behaviour, through cutting
Missed appointment with ophthalmologist (cataracts as complication from Wilson’s Disease?)
Family Hx: None disclosed.

Examination/ Investigation
Clotting studies: Normal except for prolonged PT time  Vitamin K deficiency (poor diet?)
Ultrasound of liver: Solid mass/ cancer of 5.8cm and a small degree of cirrhosis/ scarring
Presence of Kayser-Fleischer rings around corneas upon slit-lamp examination (and ceruloplasmin levels of less than 20 mg/dL in a patient with neurologic signs or symptoms suggest the diagnosis of Wilson disease)

Follow up
Pharmacologic treatment with chelating agents to bind excess copper (chelating agents and medications that block copper absorption from the GI tract are the mainstay therapy for Wilson’s disease).
Surgical resection of tumour in the liver, followed by chemotherapy (through infusion).
* Physical examination, 24-hour urinary copper excretion assay, CBC count, urinalysis, serum free copper measurement, and renal and liver function tests on a weekly basis for the first 4-6 weeks following initiation of chelation therapy. Monitor efficacy of treatment.

Note on Wilson’s disease
“Wilson disease is a rare autosomal recessive inherited disorder of copper metabolism. The condition is characterized by excessive deposition of copper in the liver, brain, and other tissues. The major physiologic aberration is excessive absorption of copper from the small intestine and decreased excretion of copper by the liver. The genetic defect, localized to chromosome arm 13q, has been shown to affect the copper-transporting adenosine triphosphatase (ATPase) gene (ATP7B) in the liver. Patients with Wilson disease usually present with liver disease during the first decade of life or with neuropsychiatric illness during the third decade. The diagnosis is confirmed by measurement of serum ceruloplasmin, urinary copper excretion, and hepatic copper content, as well as the detection of Kayser-Fleischer rings.” (eMedicine article by Rahil Shah, MD)

This is such a melancholic song-I feel sad just listening to it!..
What more when it's coupled with one of the saddest scene in House MD- Season 1, Episode 17: Role Model..when Cameron decided to leave the hospital after the "stunt" House pulled during his presentation of the new product of Volger's company..
Her "last" words: I’m protecting myself. You asked me why I like you. You’re abrasive and rude, but I figured everything you do, you do it to help people. But I was wrong. You do it because it’s right. There are only two ways I can deal with things. One is in my control. That’s to leave. Goodbye, House.

Now, blood suckers I'm talking about are not vampires..or even mosquitoes..hehe..
But, leeches!
Gross maybe, but useful they are in the medical world..
As demonstrated in Grey's Anatomy 2.11: Owner of A Lonely Heart, leeches were (and are still) used to "break up the pooling of blood" and basically to get the blood moving normally again..
The euphemism "bio-therapeutic agent" was used to introduce this miracle worker to the patient in this episode..
Anyway, thankfully Mr Murphy (the pt with the pooled blood in his nose, ears, etc post-reconstructive surgery due to multiple skin melanomas) consented to have those squiggly, slimy leeches to be put on the appropriate areas..
And leeches being leeches, they get their "y-shaped jaws and hundreds of teeth" out and start sucking the blood quite happily..
And they simply drop off once they're filled
It is some kind of miracle remedy-especially being painless and all..
Only the gross factor, and maybe fear of cross-contamination prevent medical personnel to use this as commonly as blood thinner drugs like heparin..
Back to the episode, the leeches did their job..and the guy's almost black nose and ears returned to normal shades again..Yeay!
He even kept and named those leeches afterwards..Aww!

NB: It was claimed that the use of leeches to draw blood had begun 3000 years ago in Egypt..talk about being ancient!

It's a bit ironic to think that, when the Third World countries are striving to improve their modern medicine services, the Westerners are actually going back to the wild world of complementary medicine and "bio-therapy"..
Sure some of them are real dodgy-like urine-therapy??(iyew!)..but others have shown tremendous results-as effective (sometimes even more) than modern medicine, and most importantly, much cheaper..
Anyway, to draw examples from House M.D. and Grey's Anatomy, let's go back to Season 2 of episodes 12: Distractions and 11: Owner of A Lonely Heart, respectively

Love Hurts. (ref: one of House episode in Season 1 was given this title)
It doesn't necessarily have to..but, that's the way it is-at least on screen..cos people nowadays don't just wanna see the happily-ever-after endings..instead, what they seemed to want more are tragedies..
(now, how many ppl actually stop to help when a traffic accident occurs..compared to the ppl who slow down their cars, just to have a look..later causing a massive traffic accident?[ Click here to read more ]

Hmm..this book brings back memories of my college days in Malaysia..
We had to read this as part of World Literature in our Lit class..but in the translated version..
I haven't got the chance to read the English version (soo many books, so lil time!) but I hope I will some day..
All those symbolisms in such a thin book- I simply love it

Doctors in the country?- I gather as much from the title..
Is it some part of a campaign-to encourage docs to work in rural areas with that show?hehe
Chicago Hope-hmm..only some hazy recollection cos at that time (1994ish right?) I was more into E.R..

p/s:you don't watch much of the current med shows, do you?
