One serious health problem associates with the overweight person especially the elderly one is the narrowing of one or more of the coronary arteries (blood vessels which supply blood to the muscle of your heart). This narrowing will result in insufficient blood supply to the heart muscle causing the symptoms of chest pain. In order to improve the blood flow and to relieve your pain, the artery must be widened.

One commonly used medical treatment is to insert a balloon into the part of the narrowing vessels and then inflate the balloon to expand the vessels diameter. This is called Percutaneous Transluminal Coronary Angioplasty (PTCA).

During this procedure, often time a stent is implanted in the artery. A stent is a small, tubular metal device that is placed in the narrow section of the artery and is intended to serve as a support to hold the arterial walls open. Once the stent has been implanted, it cannot be removed. As stent implantation is part of a PTCA procedure, one may not notice any extra discomfort from the additional implantation.

However, stent does not prevent a natural process which occurs over time where tissue grows into the stent. Subsequently, it narrows the treated tube at a later time. This process is known as restenosis. Fortunately, a new technology is now available to deter restenosi. This is by applying a small amount of drug to the stent to prevent or delay this building of fat.

Currently, there are three drug-eluting stents that are commercially available in the advanced medical world markets. A new deterring drug is in the final stage of testing and will be launched soon in the medical market. All of these drugs - eluting stent have the attribute of reducing the effect of re-narrowing the arteries effectively.

The Drug Eluting Stent is coated with a mixture of a biodegradable polymer and the drug Biolimus A9. The polymer [poly-lactic acid (PLA)] acts as the carrier for the drug and controls the release of the drug from the stent. PLA is a biomaterial often used in wound closure, prosthetic implant and drug delivery systems.

Treatment steps
As with any illness, we will need to seek medical help. In the case of heart complication, doctor will begin with a general medical diagnosis. This will follow with ECG check (an electrical recording of the activity of the heart.),
In a repeating visit, doctor may conduct a second ECG check. He will also obtain about 1 teaspoon of blood to do a blood test and make an appointment for an angiographic procedure.

In the subsequent visit, doctor will evaluate all the medical check-up, the ECG, the blood tests, and do the scheduled angiographic images (a film of the heart vessels). If the results show a narrowing of the coronary heart vessels, there will not be much choice but to adopt a treatment of either the traditional bypass graft surgery or the newer balloon angioplasty with a different type of stent.

The stenting operation is an extension of a balloon angioplasty procedure. Angioplasty is a technique that uses a catheter with a small balloon mounted on the end. The balloon is placed in the narrowed portion of the coronary artery and inflated. It pushes apart the narrowed walls of the artery. The stent mounted on the balloon will expand when the balloon inflates and opens up the artery that is being blocked.

Ultrasonic technique is used to direct the stent to the blocked area. It will help to detect the exact location to place the stent and inflate the balloon to open up the stent. The stent will remain expanded keeping the artery opens.

After the stent implantation, it will necessary to remain in the hospital for recuperation and further medical diagnosis with more ECG and blood test before being discharged in about 10 days time.

It is imperative to dose aspirin for an on going time under the supervision of the doctor. It is also be required to continue taking anticoagulation/antiplatelet medications (restenosi deterrent drug) for about a year. It is important that you do not miss any dosage.

A re-visit to your doctor is done in 30 days, 6 months, 12 months and every year after for ECG check and general heart evaluation.

Potential Risks
There is some element of side effects and risks in all medical treatment. Here is a list potential risk related to the stent implantation.

Risks that may be associated with a standard angioplasty procedure and the use of any stent in the coronary arteries include but are not limited to:

• Abrupt stent closure
• Acute myocardial infarction (heart attack)
• Allergic reaction
• Aneurysm (bulge in artery wall)
• Angina (chest pain)
• Arrhythmias, including ventricular fibrillation (VF) and ventricular tachycardia (VT) (irregular heart beats)
• Arteriovenous fistula (channel between artery and vein)
• Cardiac tamponade (fluid collecting in sac around heart)
• Coronary artery occlusion
• Cardiogenic shock (severe drop in blood pressure)
• Death
• Dissection (tear)
• Drug reactions to antiplatelet agents/anticoagulation agents/ contrast media
• Emboli (blockage), distal (air, tissue, or thrombotic emboli)
• Embolization, stent (blockage)
• Emergent Coronary Artery Bypass Surgery (CABG)
• Failure to deliver the stent to the intended site
• Fever
• Fistulization (abnormal communication between two vessels)
• Heart Failure
• Hematoma (bruise)
• Hemorrhage (bleeding)
• Hypotension/ Hypertension (Low Blood Pressure/High Blood Pressure)
• Incomplete stent apposition (stent not touching artery)
• Infection, including infection and/or pain at the access site
• Myocardial infarction (heart attack)
• Myocardial ischemia
• Perforation (hole) or Rupture
• Pericardial effusion (fluid collection around heart)
• Prolonged angina
• Pseudoaneurysm (outpouching of blood vessel)
• Renal failure (kidney failure)
• Respiratory failure
• Restenosis of stented segment
• Rupture of native and/or bypass graft
• Shock/Pulmonary edema (fluid in the lungs)
• Spasm
• Stent compression
• Stent migration
• Stroke/ cerebrovascular accident/ TIA
• Stent thrombosis (acute, subacute, or late)/ occlusion
• Ventricular fibrillation (irregular heart beats)
• Vessel perforation
• Vessel spasm
• Vessel trauma requiring surgical repair or reintervention

There are also potential risks that may be associated with the Biolimus A9 (drug to minimize the likelihood of clot formation at the stent site) drug coating which may include, but are not limited to the following:

• allergic/ immunologic reaction to drug (Biolimus A9 or structurally-related compounds) or the polymer stent coating (or its individual components)
• anemia (low red blood cells)
• headache
• hepatic enzyme changes
• nausea,
• hyperglycemia and mouth ulcers

The saying of “healthy is better than wealthy” may not be agreed by all, but it does contain truth and wisdom to remind us to stay healthy. But if we do fall ill, there is always a medical cure. In the case of heart artery blockage, the best medical treatment available now is enhanced stent treatment.

Massage parlors in Thailand

March 27th 2008 06:08
Massage in Thailand had its origin many years back in the palace. It was an indulgence for the senior royal members to enjoy a physical relaxation through the rubbing of the body by the young. The practice was later developed into a kind of healing for many ailments.

Modern massage in Thailand started a decade after the 2nd world war. With the increasing trades with Japan, Thailand learned to offer the Japanese style public bathing service to the ever increasing number of Japanese tourists and expatriates in Bangkok. Massage parlor or the so called Turkish bath at that time sprung up everywhere along the New Petchburi road in the early 60s. It was counted about 150 parlors along the few miles stretch of New Petchburi road at that time. This did not even include a few hundreds or so others parlors scattered around Bangkok metropolis such as the widely known Pat - Pong in Silom road and soi Texas in Sukhumvit area.

However, the name of the establishment did not actually reflect the work it supposes to render. All the massage parlors provide some type of sexual services in additional to the massage they do. Almost all customers are males of ages from 14-65. A typical standard service lasted from 2 to 3 hours. It started with a girl of your choice to give you a bath, follow with a body massage and sexual activities. The cost was from a mere US$ 0.50 to US$ 5.00 per hour depending on the type of decoration and facility of the various places then.

This type of massage parlors have evolved and upgraded into providing a full sexual service today. The charge has also gone up substantially to around US$ 50 to 100 for a two hours service session. Yet the core service remains the same. It starts with a girl of your choice to give you a bath, follow with a body massage and sexual activities.

There is also another type of massage known as Thai traditional massage in the market. It had never been attractive for a few reasons. First among them is there is seldom any sexual activities involved in this kind of massage. The second reason is all the masseuses are mature women of ages more than 35. Some of them are between 45-50 years old with over weight. Another reason is most of the places providing the services are old and sometimes unclean or unhygienic.

But with the influx of western tourists in the recent decades, traditional massage has become very popular among them. There has been a swift change in this category of massage service. New establishments are up everywhere like the blooming of the mushroom from the beaches, shop houses, department stores to the 5 stars hotel. The costs are varied depending of the place. But all are very reasonable indeed.

Thai traditional massage has never pay attention to the ears or the feet which the Chinese massage does. For the Chinese, all the veins and blood vessels eventually end at these two areas of the body. Hence, massaging these two parts of the organs freshen up the whole body. I bring up this because there is another kind of massage service in the market now called foot massage. It came with the increasing number of Chinese tourists to Thailand.

Chinese tourists came to Thailand doing a lot of walking for sight seeing and shopping that they never had done before in lives. Their legs are naturally become sore or even swollen. They do not want a body massage but a good feet massage. They introduced their homeland feet massage into the Thai market which had eventually become extremely popular among Thai recently. They are also ubiquitous and co-exist mostly with the Thai traditional massages. The cost is also similar to the traditional massages costing mostly around US$ 2-5 per hours.

There is another type of massage in Thailand came into the scene just slightly before the time of the feet massage. It is actually known as spa massage. Spa massage uses a lot of Thai herbs to massage the body. It started from a super 5 stars hotel in Bangkok about 15 years ago and spread out to other hotels and down to all sort of establishments. The spa massage comes in a full set and provides with bathing, facial, body and feet massage. It caters mainly for the well to do hi-so of local and the super rich tourist. A course of few hours spa massage can cost up to US$ 1000-2000.

So, you may see that massage can really mean anything to any person depending on what you are looking for in Thailand. It can mean prostitution in disguise or a place for socialization or a real relaxation from a good work of pressing and kneading of your body. And what ever you are looking for, we have them here in Thailand. They are all under the common trade name of unlicensed and unprotected intellectual property right “Thai massage”. So, come and enjoy.

Shakyamuni, who eventually became the lord Buddha, was born a prince in India about 500 years prior to the coming of Jesus. He was very much disturbed as to how could human being be saved after having seen all the human miseries such as sins, hatred, murders, curses and vices, diseases and deaths. He left his prince hood and wandered through India in search for a relief of these sufferings. He went through several self efforts to attain a cure including self denial and torture with no avail. In his continuing humanly search for internal peace, he claimed he attained a sudden spiritual enlightenment by certain invisible power from above. He started to preach his new found truth for many that is known of today as Buddhist teaching. In his final written word named Ramaka Buddhist Doctrines (Namada sutra in Korean), he said: "When Jesus comes, my Tao (way) will be like a lamp without oil”.

However, his posterity somehow distorted the core teachings of Shakyamuni and exploited them into setting up a religion. Buddhists in Korea have apparently been teaching that Jesus went to India 2000 years ago, received the teachings of Buddha, and went back to give rise to Christianity. However, if this was true, there should be the teaching of Shakyamuni in the Bible. But the truth is there is no evidence of any Buddhist teaching in the entire book of the word of God.

In the Korean Buddhist teachings, there are the books of Chunsoo, the Keumkang, the Prajna-para-mita, the Avatamska, Tripitaka, and Ramaka Buddhist Doctrines (Namada). In the Namada which is known as the last teaching of lord Buddha, he prophesized the coming of Jesus in about 500 years later. Namada teaching (38:8) recorded that his doctrines would all be useless when Jesus comes. The verse that is known as the last verse in all the books of Shkyamuni is stated, "When Jesus comes, and my Tao (way) will be like a lamp without oil.

It means that "when Jesus comes, my spiritual enlightenment will become useless like a lamp without oil." It is his statement in his last sermon at the very last moment of his life. It means Shakyamuni was fully aware that his teachings should cease when Jesus comes. For Jesus alone are the way, the truth, and the life (John 14:6). Salvation is found in no one else, for there is no other name under heaven given to men by whom we must be saved, Acts 4:12.

This same Buddhist prophecy about Jesus is also found in Japanese Buddhist scriptures of Daisho Shinsoo Tripitaka. This copy, in fact, is the original of the Korean Namada sutra version. It was translated by the renowned Japanese Buddhist priest Sung-gyun of Chin Kingdom from the India Sanskrit and Bali scriptures, and were compared, edited against the Chinese text, and circulated among temple scholars for final approval before printing.

“When Jesus comes, my Tao (way) will be like a lamp without oil” is not just another saying but an actual scripture found in many versions of Buddhist teaching including the firsthand copies of Sanskrit and Bali.

An attractive church means a church that members love to attend church meetings and activities. It also means members would invite other family members and friends to attend the church he/she is attracted. This is a result of a church that is healthy, vibrant and growing or attractive in short.

An attractive church is certainly not a church that is stagnant or dying but a lively and growing church. Growing and attractive are indeed two names of the same church attribute. Both have the same distinct characteristics. And when we look at them closely, we can learn of the secret behind. We may apply them to make our church attractive and grow

Thai politics – now what?

March 2nd 2008 09:04
The former Thai Premier Taksin Shinawatana has returned to Thailand to fight his corruption cases on February 26, 2008. Will his return be developed into a social turmoil as many have feared?

The day on his return, CNN and Reuter reported that Taksin will certainly not abandon politics as he had vowed in many occasions. Foreign news media apparently raised a question as to whether Thailand will now have two Premiers concurrently. Samak Soontorawed, the elected Premier, has been regarded as a nominee or a puppet of Taksin. Samak took the interrogation as an insult and blasted out in his habitual blatantly style when answering a reporter’s question as being senseless

Tip in coin collection

February 24th 2008 14:41
I started collecting coins early at the age of 10. I was not really interested in coins then. But I was just being stirred by my brother who was already a serious coin collector. Like all kids, I was simply a copy cat in coin.

When I grew and traveled the world, I found that I always had some loose change when I left a country. I decided to collect coins and keep that hobby for over 30 years now. Throughout the years, I had learned some coins collection tips

What it means to be Asian?

February 23rd 2008 04:47

It means one thing to one and another thing to other. It may mean being an Indonesian to one and being Japanese to the other. It may mean Buddhism or Confucius. Or it may mean black eyes, black hair or brown skin. The west looks at Asian with a suspicious tone. Hence the saying, “East is east, west is west and the twain shall never be met.” Even Asian does not quite fully understand her own self and hence this question of, “What it means to be Asian?”

Comparing the Chinese and Japanese cultures

The word Japan "-" is not written in the Japanese Hiragana or Katakana but Chinese Kanji. “Kan” or “-” is the biggest Chinese tribe in China. China is made up of one big tribe and 4 small tribes as represented in the one big star and 4 small surrounding stars on the Chinese red five stars flag. “Ji” or “-” means Chinese characters. Why would “Japan” pen himself as a Chinese tribe is not without a reason

Will we meet our pet in heaven?

Will we meet our pet in heaven?

I remember the day our dog died after having him with us for 8 and a half year. My wife wept for days after we had a simple funeral for him in front of our yard. She kept asking me if we would see him again before Him. Meanwhile we sang "God be with you till we meet again
