One serious health problem associates with the overweight person especially the elderly one is the narrowing of one or more of the coronary arteries (blood vessels which supply blood to the muscle of your heart). This narrowing will result in insufficient blood supply to the heart muscle causing the symptoms of chest pain. In order to improve the blood flow and to relieve your pain, the artery must be widened.

One commonly used medical treatment is to insert a balloon into the part of the narrowing vessels and then inflate the balloon to expand the vessels diameter. This is called Percutaneous Transluminal Coronary Angioplasty (PTCA).


During this procedure, often time a stent is implanted in the artery. A stent is a small, tubular metal device that is placed in the narrow section of the artery and is intended to serve as a support to hold the arterial walls open. Once the stent has been implanted, it cannot be removed. As stent implantation is part of a PTCA procedure, one may not notice any extra discomfort from the additional implantation.

However, stent does not prevent a natural process which occurs over time where tissue grows into the stent. Subsequently, it narrows the treated tube at a later time. This process is known as restenosis. Fortunately, a new technology is now available to deter restenosi. This is by applying a small amount of drug to the stent to prevent or delay this building of fat.

Currently, there are three drug-eluting stents that are commercially available in the advanced medical world markets. A new deterring drug is in the final stage of testing and will be launched soon in the medical market. All of these drugs - eluting stent have the attribute of reducing the effect of re-narrowing the arteries effectively.


The Drug Eluting Stent is coated with a mixture of a biodegradable polymer and the drug Biolimus A9. The polymer [poly-lactic acid (PLA)] acts as the carrier for the drug and controls the release of the drug from the stent. PLA is a biomaterial often used in wound closure, prosthetic implant and drug delivery systems.

Treatment steps
As with any illness, we will need to seek medical help. In the case of heart complication, doctor will begin with a general medical diagnosis. This will follow with ECG check (an electrical recording of the activity of the heart.),
In a repeating visit, doctor may conduct a second ECG check. He will also obtain about 1 teaspoon of blood to do a blood test and make an appointment for an angiographic procedure.

In the subsequent visit, doctor will evaluate all the medical check-up, the ECG, the blood tests, and do the scheduled angiographic images (a film of the heart vessels). If the results show a narrowing of the coronary heart vessels, there will not be much choice but to adopt a treatment of either the traditional bypass graft surgery or the newer balloon angioplasty with a different type of stent.

The stenting operation is an extension of a balloon angioplasty procedure. Angioplasty is a technique that uses a catheter with a small balloon mounted on the end. The balloon is placed in the narrowed portion of the coronary artery and inflated. It pushes apart the narrowed walls of the artery. The stent mounted on the balloon will expand when the balloon inflates and opens up the artery that is being blocked.

Ultrasonic technique is used to direct the stent to the blocked area. It will help to detect the exact location to place the stent and inflate the balloon to open up the stent. The stent will remain expanded keeping the artery opens.

After the stent implantation, it will necessary to remain in the hospital for recuperation and further medical diagnosis with more ECG and blood test before being discharged in about 10 days time.

It is imperative to dose aspirin for an on going time under the supervision of the doctor. It is also be required to continue taking anticoagulation/antiplatelet medications (restenosi deterrent drug) for about a year. It is important that you do not miss any dosage.

A re-visit to your doctor is done in 30 days, 6 months, 12 months and every year after for ECG check and general heart evaluation.

Potential Risks
There is some element of side effects and risks in all medical treatment. Here is a list potential risk related to the stent implantation.

Risks that may be associated with a standard angioplasty procedure and the use of any stent in the coronary arteries include but are not limited to:

• Abrupt stent closure
• Acute myocardial infarction (heart attack)
• Allergic reaction
• Aneurysm (bulge in artery wall)
• Angina (chest pain)
• Arrhythmias, including ventricular fibrillation (VF) and ventricular tachycardia (VT) (irregular heart beats)
• Arteriovenous fistula (channel between artery and vein)
• Cardiac tamponade (fluid collecting in sac around heart)
• Coronary artery occlusion
• Cardiogenic shock (severe drop in blood pressure)
• Death
• Dissection (tear)
• Drug reactions to antiplatelet agents/anticoagulation agents/ contrast media
• Emboli (blockage), distal (air, tissue, or thrombotic emboli)
• Embolization, stent (blockage)
• Emergent Coronary Artery Bypass Surgery (CABG)
• Failure to deliver the stent to the intended site
• Fever
• Fistulization (abnormal communication between two vessels)
• Heart Failure
• Hematoma (bruise)
• Hemorrhage (bleeding)
• Hypotension/ Hypertension (Low Blood Pressure/High Blood Pressure)
• Incomplete stent apposition (stent not touching artery)
• Infection, including infection and/or pain at the access site
• Myocardial infarction (heart attack)
• Myocardial ischemia
• Perforation (hole) or Rupture
• Pericardial effusion (fluid collection around heart)
• Prolonged angina
• Pseudoaneurysm (outpouching of blood vessel)
• Renal failure (kidney failure)
• Respiratory failure
• Restenosis of stented segment
• Rupture of native and/or bypass graft
• Shock/Pulmonary edema (fluid in the lungs)
• Spasm
• Stent compression
• Stent migration
• Stroke/ cerebrovascular accident/ TIA
• Stent thrombosis (acute, subacute, or late)/ occlusion
• Ventricular fibrillation (irregular heart beats)
• Vessel perforation
• Vessel spasm
• Vessel trauma requiring surgical repair or reintervention

There are also potential risks that may be associated with the Biolimus A9 (drug to minimize the likelihood of clot formation at the stent site) drug coating which may include, but are not limited to the following:

• allergic/ immunologic reaction to drug (Biolimus A9 or structurally-related compounds) or the polymer stent coating (or its individual components)
• anemia (low red blood cells)
• headache
• hepatic enzyme changes
• nausea,
• hyperglycemia and mouth ulcers

The saying of “healthy is better than wealthy” may not be agreed by all, but it does contain truth and wisdom to remind us to stay healthy. But if we do fall ill, there is always a medical cure. In the case of heart artery blockage, the best medical treatment available now is enhanced stent treatment.

An attractive church means a church that members love to attend church meetings and activities. It also means members would invite other family members and friends to attend the church he/she is attracted. This is a result of a church that is healthy, vibrant and growing or attractive in short.

An attractive church is certainly not a church that is stagnant or dying but a lively and growing church. Growing and attractive are indeed two names of the same church attribute. Both have the same distinct characteristics. And when we look at them closely, we can learn of the secret behind. We may apply them to make our church attractive and grow.

When we make a quick survey of some of the healthy churches in USA and around the world, we see they all have some common traits that make their church attractive. They may be stated in brief as tips below:

Tip number one
This is not only the first tip in priority but it is also the most important factor to have an attractive church. For all churches around the world I have studied and experienced regardless of the denomination, the primary catalytic factor for their attractiveness is the pastor. Look at the Yoido Full Gospel Church in Korea that attracts about one million members, Pastor Cho is certainly a thinker, dynamic and command a high degree of love and esteem for their leader.

I have also observed church life in Baptist, Presbyterians and Pentecostal. I have seen it in large church (about 10,000 members in Thailand) and small church, Burmese, Chinese, International churches. The pattern of an attractive church is pastor who is well loved through living relationships with the people.

Tip number two
Dr. Rick Warren, senior pastor of Saddleback Valley Community Church in Orange County, tells of his life experience of an attractive church in his landmark book “The Purpose Driven Church” that such a church is driven with a purpose. Life is a purposeful life and it would be dull if we attend a church without a purpose. How boring would it be if we have to go through the same rutted church life day in day out.

It may be true that every church is driven by something of tradition, finance, programs, events, and even building. But Rick Warren believes that in order for a church to be healthy, it must become a purpose – driven church build around the five New Testament purposes given to the church by Jesus. And Jesus told us the five are:
- Warmer through fellowship.
- Deeper through discipleship
- Stronger through worship
- Broader through ministry
- And Larger through evangelism.

Tip number three
For a church to be attractive, it must motivate and involve all members in any of the five church purposes in tip number two. Pastor of attractive churches, whether they be large or small, know how to motivate their members to be active and productive and guide them into meaningful avenues of Christians services.

Motivation should be done from church leaders and spread to every one in the church. Pastor Cho managed to have some of his church members to spare 15-20 minutes a day helping mothers carrying things with babies and the seniors getting in and out of the high rise lifts and witness Christ to them.

Tip number four
Dr. Donald McGavran, in his classical text “Understanding Church Growth”, indicates that people like to become Christians without crossing racial, social, linguistic and class barrier. The term “homogeneous unit” is coined to mean a group of people who consider each other to be “our kind of people”. They have many areas of mutual interest. They share the same culture. They socialize freely. When they are together they are comfortable and they all feel at home.

The church should encourage members into different homogeneous units. They may be units of sex, age, race, occupation, hobbies and so on.

Tip number five
Dr. C. Peter Wagner has been a professor at the Fuller Theological Seminary School of World Mission since 1971 and wrote more than a dozen of books in church growth. He has studied, analyzed, and diagnosed churches for more than 20 years and concluded that one of the major character of an attractive church is the way how they worship.

People can worship God beside a babbling brook in a forest or in his car on a freeway or with his family around the dinner table. But Wagner considers that to attract members to worship, the church should do it in three different levels of celebration, congregational and cell worships.

By celebration, he means Sunday worship when a lot of people come together to experience a special kind of worship. Size has something to do with the quality of a celebration as a game played before seventy-five thousand spectators makes it superior to the same game before a mere five hundred.

However, a good celebration alone does not make for an attractive church. It must also be balanced with congregational and cell worships. Both are medium and small groups worship. Worship is closely related with eye contact of personal fellowship with each other.

Personally, I have experienced the warmth and attractiveness of churches with these characteristics. I am positive that the five tips will work well in your church to become an attractive church.

Tip in coin collection

I started collecting coins early at the age of 10. I was not really interested in coins then. But I was just being stirred by my brother who was already a serious coin collector. Like all kids, I was simply a copy cat in coin.

When I grew and traveled the world, I found that I always had some loose change when I left a country. I decided to collect coins and keep that hobby for over 30 years now. Throughout the years, I had learned some coins collection tips.

Coins cleaning
When I came across an old coin with heavy stain or rust like layer due to chemical reaction to the metal in my early collection time, I simply polish the coin with Braso. But I found a few very annoying things in doing so. First, it takes a long time to clean up a coin from hours to days depends on the nature of dirt.

Second, I could never clean a coin satisfactory. There will always be a good portion of the coin that can not be polished or cleaned. And with the number of coins I started to accumulate, I had a hard time trying to hand polish some rusted coins. I started to experiment coin cleaning with various chemicals and I struck out one excellent cleaning agent from my own washroom cleaning product.

The trade name in Thailand is Vixol and the active ingredients are Hydrochloric acid, Ethoxylated Nonylphenol, and Linear Alkylbrnzene Sulfuric acid. I would normally pour the liquid directly on the coin until it covers. The liquid will react only with the stain without harming the metal. Minutes later, it will clean the coin thoroughly. Rinse in clean water and wipe dry. You will now have a spotless clean coin.

Coin Storing
The next step is to keep the coin in this cleaned condition for as long as possible. To do so, we need to cover it with a coat of sealant. I use both floor sealant and lacquer for the purpose. But we need to apply with the transparent type and dull finish. Glossy finish will make the coin looks artificial suggesting a fake nature.

Invest in good storage coin books. This will prevent premature wear and tear and subsequent unnecessary damage to coins. Also categorize according to types such as country, year minted, metal content (Gold or silver or nickel) before arranging into the books. Always allow ample emptied spaces in between your arrangement so that you can insert some new coins in between category without having to rearrange the whole book.

Coin verification
The old saying that, “If you keep looking at the Genuine long enough, you will be able to spot the fake one immediately” may not be all true in checking coin. Some fake coins are indeed hard to distinguish. But there is a simple rule of thump technique that works almost 100 % in detecting fake coin.

Lay the coin flat on a table preferably a glass top table and give it a hard spin. If the coin stops after a round of spin or less, it is genuine. Otherwise if it keeps on spinning for several rounds, there is almost certain that it is faked. If the cost is worth more than a US$ 1000, get a second or third opinion to check. If it costs is over US$ 10,000, I would play safe to have a laboratory verification.

Having all said, I would like to add that I have just sold three coins I bought for 10 Bahts (US$ 0.30) in 1987 for 2560 Bahts (US$ 80) a piece recently. I also have coins worth over US$ 10,000 I am still reluctant to part with. So, I would say it is not bad as a reward for a hobby I enjoy.
